Test Proctoring Request Process

In order to offer DSS and your professors the appropriate amount of time to coordinate your exam accommodations, you must secure your space by submitting a test proctoring request, at least seven (7) days in advance of the examination date. The following steps help to ensure your exams are properly administered with your accommodations:

In order to offer DSS and your professors the appropriate amount of time to coordinate your exam accommodations, you must secure your space by submitting a test proctoring request, at least seven (7) days in advance of the examination date. The following steps help to ensure your exams are properly administered with your accommodations: 

Steps to Submit Your Requests

  1. Log in to the Test Proctoring Form with your GW UserID and password. Your log in identity should be consistent with what is used to access your GW email, Blackboard, and other related applications. If you have any issues accessing the form, please notify [email protected] immediately.
  2. Check to ensure your name and email are accurate and fill in the required fields for your professor’s contact information. If you are unsure about your professor’s contact information, please check your syllabus or the GW Directory to confirm.
  3. When indicating the start and end time under the "Course Information" section, please include the times for the class as listed on the University Schedule
  4. The "Test Information" section requires you to indicate when you will take your exam. The start time for your exam should match the start time for the class in question, and the test date should fall on a day when your class meets.

    In some cases, your test information will not directly correspond with the course information. Use the "Note to Professor" section to notify your professor if any of these discrepancies occur. The two instances in which we will allow for this discrepancy are listed below:
    1. If you must begin at an earlier or later time to avoid an extended time conflict with another class or another exam.
    2. If your exam is originally scheduled outside of or will go beyond: 
      Regular Test Proctoring Hours (Monday - Friday, 8:30 am - 5:30 pm) or 
      Finals Week Test Proctoring Hours (Monday - Friday, 8:30 am - 6:30 pm)
  5. Under the "Accommodations Required," section check only the accommodations for which you have been approved. We will cross check your requests for accommodations with our records. Note: If you need to request additional accommodations outside of your currently approved, you will need to contact your DSS representative directly for review.
  6. We highly recommend completing the "Note to Professor" section as a general courtesy and as a reminder of your DSS registration. This section is also the place where you can alert your professor to notify us of any unique exam instructions (e.g. use of notes, calculator, scratch paper, open book exam, etc.).
  7. The required checkbox at the bottom of the form certifies that you have discussed your approved accommodations with your professors and developed a plan for activating those accommodations in each of your classes, in addition to sharing your Letters to Professors. It also certifies your compliance with GW’s Code of Academic Integrity.
  8. Choose "Submit Request" to proceed to a review screen that will list all of the details that you have entered. Double-check all of the information for accuracy before submitting; choose "Edit Test Proctoring Request" if you need to make any changes.
  9. Once you have verified your exam details on the review screen, choose "Submit Test Proctoring Request" to complete the process. Keep in mind that you must submit a test proctoring request for each quiz and exam you wish to have proctored by DSS.
  10. Email confirmations are sent promptly to your GW email address after requests are officially submitted. Please save a copy of this confirmation for your records. Your professor will also receive an email notification of your request and will either approve or deny the request, or possibly alter it (e.g. approve the use of a calculator) to make it more precise. Note: You will receive an email notification if your request has been approved, denied or altered. Throughout the semester, please make sure you review each result in detail before arriving at your test proctoring location. Scroll all the way down to check for any changes made by your professor. Contact your professor if you have any questions or concerns about the results of your test proctoring request.
Late Request Consideration

If you forget to submit a test proctoring request at least seven (7) days in advance of your exam date, you will need to submit your request formally to [email protected] and copy your professor. In doing so, you must include the following details (incomplete requests may not be considered):

  • Professor’s name
  • Course title, number and section
  • Exam day and time
  • Brief explanation regarding why your request is late
  • Be sure to copy your professor on ALL late requests

We recognize that mistakes are inevitable, however we must adhere to a consistent, smooth scheduling process for the benefit of all the students and professors who coordinate exams with our office. We will make decisions about late requests on a case-by-case basis.
