Guidelines for Submitting Documentation
At Disability Support Services (DSS), we carefully review all submitted documentation to understand how your disability affects you in an academic setting. This documentation helps us determine the most appropriate accommodations based on your current needs. To ensure timely processing, we recommend submitting your documentation well in advance of any accommodation-related deadlines.
How to Submit Documentation
You have several options for providing DSS with the necessary documentation to verify your disability. Documentation should clearly confirm your diagnosis. Some examples of acceptable documentation include:
- A completed Disability Verification Form
- A comprehensive evaluation or diagnostic report
- An accommodation letter from a previous post-secondary institution
- A letter from a healthcare professional on letterhead, including a signature, confirming your diagnosis
DSS may ask you to provide additional information if the submitted documentation is incomplete or does not support the requested accommodations.
